Former Stone Temple Pilots singer Scott Weiland passed away earlier this month while on tour in Minnesota, prompting an outpouring of remembrance from friends and fans and an honest, poignant obituary from Weiland’s ex-wife and children.

Today, as Rolling Stone reports, the cause of death was confirmed as an accidental overdose resulting from a mixture of cocaine, ethanol and methylenedioxyamphetamine (MDA), according to the Hennepin County Medical Examiner in Minneapolis.

Weiland had struggled with addiction and drug abuse throughout his life. A small amount of cocaine was found in the tour bus where he was found, and shortly after his death, the bassist for his band the Wildabouts, Tommy Black, was briefly arrested for possession of the drug.
